SPORTS PHYSICAL EXAMINATION INFORMATION

If your child is interested in participating in interscholastic sports at Indian Trail this coming school year, they will need an Interscholastic Sports Physical by a physician.  Sports physicals are valid for one year from the date of the physical.  You may download the official Health Examination form or receive a hard copy from the Indian Trail office in August.

Students planning to participate in cross country (all 6th through 8th grade students are eligible to participate) need to turn in a completed physical form before the first meet. Please plan to make an appointment for a physical exam before the school year begins.
